Action plan Project leads have been appointed in each area A quality improvement steering group monitors the milestones for the project plan and reports to the Western Health Best Care – Coordinated Care sub committee. Gap analysis for all Specialist outpatient clinics has been completed
Action Plan continued Action Plans will now be developed for the target process not being currently met Referral reports are available within Western Health reporting systems
Challenges Additional process – requires additional or reconfiguration of the same administration FTE Change management Western Health needs changes to the dictation software to be able to report on targets for communication (10.3 and 11.2) Without data for 10.3 and 11.2 we do not know what out gap is.
Challenges Meeting triage time frames Clinics that do not run weekly No back fill for clinics/specialty Electronic referral and triage High volume clinics and discharge summaries
