A new PI, Dr. Jekyll, has an approved protocol to conduct a pilot study. The pilot study was completed, and the results were not reported to the IACUC. The IACUC also did not follow up with Dr. Jekyll on the results of the pilot study. The AV also stated that she has not been able to enter Dr. Jekyll's animal room due to the door being locked at the time of her scheduled checkups. Case study #1
Dr. Jekyll has IACUC approval for non-standard husbandry for his rabbit study. Animal care staff noted that the person responsible for conducting the procedures, Mr. Hyde, is not properly documenting the non-standard feed and water schedules. Concurrently, the IACUC coordinator discovered that Mr. Hyde does not have documentation of his animal care training, but claims to have completed it. Case study #2

1. Choose the best definition of program review: • Semiannual review of the activities conducted by and at an institution that have a direct impact on the well-being of animals. • Semiannual review of the functions of the IACUC. • Semiannual review of veterinary care, occupational health and safety, and animal facility design. 2. Case Study – Identify and Classify any Deficiencies. Animal care staff noted that Dr. Jekyll did not report minor lesions found on the rabbits in his study. One of the animal care staff members tried to report the minor lesions that were discovered, but was not able to find posted animal welfare reporting contact information. Furthermore, they reported that non-pharmaceutical grade chemicals were in use and had not been approved by the IACUC. As a result, Dr. Jekyll was terminated, Mr. Hyde was demoted, and it was discovered that the non-affiliated IACUC member was on the board of the company sponsoring the rabbit project. Summative assessment
3. Classify these deficiencies as Minor (M) or Significant (S): __No backup veterinarian is available. __ Animals of different species are quarantined together. __Annual zoonotic training was not completed within the specified timeframe. __Semiannual program review was not completed in the specified timeframe. Summative assessment
