Provide documentation Requirements A diagnostic statement identifying the disability, date of most current evaluation, and date of original diagnosis A description of the diagnostic tests or criteria used Treatments, medications, and/or assistive devices currently prescribed or in use A description of the functional impact of the disability Most recent Individual Education Plan (IEP)

    6. Services Available All students are expected to meet the standards of conduct, attendance, and course requirements of the college. Accommodations are determined based on the nature of the disability. They may include, but are not limited to the following:

Kurzweil (screen reader) Note-takers, interpreters, and/or scribes Adaptive equipment for classrooms and computers Dictation Software Test-taking adjustments/facilities Tape recording lectures Audio enhancing devices Text enlargement Readers for exams Enlarged computer screen Track ball mouse Books on tape Enlarged print Screen filter Tutoring Supportive Counseling

    12. Checklist Provide professional documentation Contact the LSC for accommodations before taking the ASSET/COMPASS Test. Discuss accommodations with instructors. Keep the LSC updated regarding accommodation requests. The student needs to provide a copy of his/her schedule each semester to the LSC so instructors can be notified. Concerns about accommodations or services should be addressed to the LSC immediately.

    16. Cosmetology Completion qualifies student to take a state exam for registration as a licensed cosmetologist in the state of Illinois.

    17. Administrative Information Tech Entry level assistant position Emphasis is placed on computer use and office technologies Good work habits and attitudes Jobs in banking, education, public relations, law, government, medicine, retail, and industry

    18. Massage Therapy Work in the wellness area of professional massage therapy Completion qualifies students to sit for the National Certification Exam in Therapeutic Massage and Bodywork

    19. Computer Networking Specialist Advanced Networking Specialist Computer networking specialists and systems administrators provide technical assistance, support and advice to customers and other users Interpret problems and provide technical support for hardware, software and systems

    20. Medical Transcription Medical transcriptionists, medical receptionists, and other related personnel Jobs available in hospitals, clinics, doctors’ offices, insurance companies, health foundations, local industries and Illinois state and U.S. government agencies

    21. Paraprofessional Educator For current paraprofessional educators with 30+ postsecondary credit hours NCLB requirements Degree: teachers aide, special ed aide for K-12, pre-school aide, and aides for child/day cares
