1. Call the LSC • To make an appointment 395-7777, ext 2264 or 2265
2. Provide documentation • Requirements • A diagnostic statement identifying the disability, date of most current evaluation, and date of original diagnosis • A description of the diagnostic tests or criteria used • Treatments, medications, and/or assistive devices currently prescribed or in use • A description of the functional impact of the disability • Most recent Individual Education Plan (IEP)
3. Meet with Donita or Peg • Donita Kaare, LSC Director • Peg Kennedy, Developmental Support Specialist
Services Available • All students are expected to meet the standards of conduct, attendance, and course requirements of the college. • Accommodations are determined based on the nature of the disability. They may include, but are not limited to the following:
Kurzweil (screen reader) • Note-takers, interpreters, and/or scribes • Adaptive equipment for classrooms and computers • Dictation Software • Test-taking adjustments/facilities • Tape recording lectures • Audio enhancing devices • Text enlargement • Readers for exams • Enlarged computer screen • Track ball mouse • Books on tape • Enlarged print • Screen filter • Tutoring • Supportive Counseling
Accommodations will not include: • Personal attendants • Special transportation • Readers for personal use • Anything that would fundamentally alter the nature or essential elements of a course or program

6. Appointment with Advisor • Make appointment with Advisor in Student Services for Registration
Checklist • Provide professional documentation • Contact the LSC for accommodations before taking the ASSET/COMPASS Test. • Discuss accommodations with instructors. • Keep the LSC updated regarding accommodation requests. • The student needs to provide a copy of his/her schedule each semester to the LSC so instructors can be notified. • Concerns about accommodations or services should be addressed to the LSC immediately.

Automotive Service Technology I & II • Technician in general auto repair • Jobs available in this area include auto technicians, dealerships, garages, auto specialty shops, and parts-related businesses.
Basic Nurse Assistant Training • Intro to basic components of health care skills essential to support and assist in meeting basic human needs of families and individuals of all ages. • Employment in long term care facilities or home health-care situations.
Cosmetology • Completion qualifies student to take a state exam for registration as a licensed cosmetologist in the state of Illinois.
Administrative Information Tech • Entry level assistant position • Emphasis is placed on computer use and office technologies • Good work habits and attitudes • Jobs in banking, education, public relations, law, government, medicine, retail, and industry
Massage Therapy • Work in the wellness area of professional massage therapy • Completion qualifies students to sit for the National Certification Exam in Therapeutic Massage and Bodywork
Computer Networking SpecialistAdvanced Networking Specialist • Computer networking specialists and systems administrators provide technical assistance, support and advice to customers and other users • Interpret problems and provide technical support for hardware, software and systems
Medical Transcription • Medical transcriptionists, medical receptionists, and other related personnel • Jobs available in hospitals, clinics, doctors’ offices, insurance companies, health foundations, local industries and Illinois state and U.S. government agencies
Paraprofessional Educator • For current paraprofessional educators with 30+ postsecondary credit hours • NCLB requirements • Degree: teachers aide, special ed aide for K-12, pre-school aide, and aides for child/day cares
Phlebotomy Certificate • Learn techniques for the collection of blood from patients for diagnostic testing. • Ethical and legal responsibilities, communication skills and safe practices are studied. • Employment opportunities are in hospitals, laboratories, health care providers
WVC or FCC only Psychiatric Rehabilitation • Psychiatric support personnel in public, private, and government-operated facilities • Psychiatric technicians and aides working in settings, such as mental health facilities, public and private hospitals, residential programs, and support services programs.
Web Design • Web page creation for business, industry, and personal use
Welding and Cutting • Training welding, cutting, and burning • Jobs available in local industries, construction, oil field work, private enterprises, and farming
