3. PICH History • Established 2011 • To authentically engage medically underserved communities in addressing health disparities. By establishing meaningful relationships between students, health professionals, community leaders, and citizens, we will work together for sustainable solutions.

6. 2013-2014 Internships • We would like to maintain our partnership with these organizations. We ask that you follow the following guidelines: • Feel free to contact partners for more information. However, please follow through when you contact a partner. Think carefully about your schedule and interests before you commit to a project/partner. • Please let us know where and in what capacity you start volunteering for our records. • Please fill out a log when you volunteer. You can find logs under the Member Portal tab on our website. Additionally if you choose to keep a log yourself, please share your hours with PICH every semester (pich.umntc@gmail.com). • We would like to get an idea of how many volunteer hours go through PICH to determine to effectiveness of our program. • Please let us know if there are problems with your site so that we can address them as soon as possible.

8. Community University Health Care Center (CUHCC) • Who they serve: Community-University Health Care Center (CUHCC) opened its doors in 1966 to  provide primary care services to children and low income families in South Minneapolis to provide excellent community health care for diverse populations. They work with Spanish, Cambodian, Hmong/Lao, Vietnamese and Somali translators.   Volunteer Opportunities  • Provide Communications/Clinic OutreachAttend community health fairs and events, and maintain distribution tracking list, Distribute family planning materials to various affordable housing units surrounding CUHCC and to staff of about 25‐50 health, housing employment and human service agencies (track distribution list). • “Welcoming presence” in Lobby Assist patients as they get into lines for check‐in. Assist staff who are getting patients from waiting room into clinical areas, by finding the patient they’re trying to room. Assist patients with routine health history questionnaires as they wait to be called to clinical areas. Provide in‐person messaging link between front desk and clinical areas, upon on request from CUHCC staff. Read to children. Hand out health education materials from a desk or table in the lobby. Periodically: hand out short surveys to patients as they sit in lobby. • “ Cycle time recorder” for patient visits Discreetly monitor a sampling of arriving patients while sitting in the lobby: note what time they check‐in, when they go in and out of registration, and finally into clinical area. We will identify the patients to monitor by asking them to carry a color‐coded card, or similar. Focus is on the afternoon clinics, prefer the volunteer would be on site no later than 1 p.m. and remain for an hour or two. • Staff Surveyor: Construct and tabulate staff engagement surveys Timing - occasional. Can be done in conjunction with either #1 or #2 roles above. Using Survey Monkey or similar, send out and tabulate results, content to be determined. Occasionally ask follow‐up questions in‐person 1:1. • Patient surveyor Conduct (either in lobby or by phone) patient satisfaction surveys for all clinic areas, and assist in tabulating survey results. (Excel skills required. Bi‐lingual in Spanish, Somali, Hmong, Lao, Cambodian, or Vietnamese preferred.)

9. Fairview Home Care and Hospice (FHCH) • Who they serve: Fairview Hospice has provided end-of-life care for more than 25 years.  Volunteer opportunities: • Patient/Caregiver Support Volunteers Provide support and comfort by helping with the practical needs of patients and their caregivers. • Check-In Call Volunteers Provide support and comfort through weekly phone calls to patients and caregivers to make sure they have everything they need before the weekend. • Massage Therapy Volunteers Provide relaxation and pain relief to patients through full or partial body massage. To enhance the care provided by Fairview massage therapy employees. • Energy Work Volunteers Provide relaxation, calming, and pain relief to patients through energy work • 11th Hour Volunteers Provide companionship and support in the final 24-72 hours of life • We Honor Veterans (WHV) Volunteers Provide end of life care that meets the unique needs of veterans and to recognize them for their service to our country. • Do Re MiVolunteers Provide enjoyment and relaxation through playing musical instruments and/or singing to patients/caregivers

10. Family Education Diabetes Series (FEDS) • Who They Serve: Primarily older, Native American people from the twin cities area who are concerned about developing diabetes or who already have. The Saint Paul Area Council of Churches and the Department of Indian Work help to present a dinner series open to anyone where attendees can get their blood pressure, weight, BMI and blood glucose screened, have a healthy dinner, and have presentations about diabetes.  Volunteers Opportunities • Education SeriesTwo med students who can switch off on making 5-10 minute presentations about aspects of life with diabetes, and who can help with the screenings and with serving the dinner.  Dinner is every other Tuesday, 5:30-7:30. 

11. Neighborhood HealthSource – Fremont Clinic • Who they serve: Neighborhood HealthSource has provided quality, affordable health care to our community for more than 40 years. Neighborhood HealthSource started as two small, "free clinics" — Beltrami Health Center (now Central Clinic) and Fremont Clinic, staffed by volunteers and operating out of make shift rooms in community centers. The clinics incorporated in 1971, in the early '80s, bought and renovated their facilities and merged in 1988 to form Fremont Community Clinics, Inc. In 1995 we opened Sheridan Clinic in Northeast Minneapolis. Volunteer opportunities: • Billing Office Volunteer Work directly with our Billing Office Director to assist our billing department by ensuring that patients receive billing mailings. Use of Electronic Medical Records and patient phone calls. Support the billing department and Director through other various administrative projects, including updating contact information, and determining financial  eligibility for clinic services. • Clinical Support Volunteer Work with our outreach and administrative staff to support a wide variety of clinic projects as needed. Sort donations of books for our children’s reading program. Assist with event planning and fundraising. Make appointment reminder phone calls to patients. Prepare mailings. Pull charts  • Community Outreach Representative Assist our Community Health Worker for our various outreach projects to conduct outreach work in the communities of North and Northeast Minneapolis. Talk one on one with community members about Neighborhood HealthSource’s services and outreach programs .Answer questions. Provide further resources • Fundraising Volunteerresearch community opportunities, write letters and secure donations for our 9th Annual Gala!, participate in grant writing activities, Participate in capital campaigns.  • Medical Administrative Assistant Volunteer Work with our Medical Director to support a wide variety of clinical projects. Work collaboratively with Neighborhood HealthSource staff to achieve optimal office efficiency. Taxing Filing Photocopying Proofreading documents Managing follow-up calls respectfully and promptly.   • Quality Improvement Volunteer Work directly with the Quality Analyst. Support our quality improvement initiatives,  Assisting the quality analyst with the clinical quality audits for our summer measures.  Assisting the QA with monthly diabetes and hypertension performance audits for distribution to providers  Work with the QA and providers/MAS to develop and implement action plans for improvements in colorectal cancer, adult depression, and breast cancer screening rates.  Assist in the development of a "quick reference" chart of required documentation for use by medical providers.  Other Projects and Tasks as needed 

12. Native American Community Clinic (NACC) • Who they serve: Native American families in the metropolitan area who cannot afford health care. Volunteer Opportunities • Fitness Center Attendants: Health education, Assist manager and fitness instructors, Offer health education classes and at health fairs

NorthPoint Health and Wellness Center • Who they serve:NorthPoint strives to improve the physical and socio-economic health of the North Minneapolis community through an integrated model of health and human services. Volunteer Positions • Mobile Food Shelf VolunteerVolunterat Food Shelf to get a sense of how the Food Shelf operates. Identify and expand the Mobile Food Shelf Project to include more low-income senior housing/apartment around North Minneapolis.-Develop nutrition education materials either from original or already developed sources.-Develop surveys to assess client needs. Work with Food Shelf staff to develop a Recipe Books for distribution at the Food Shelf. Go with the Mobile Food Shelf on Friday to distribute foods and present nutrition information to clients.  • Wellness Group FacilitatorReview the General Wellness curriculum. Organize focus groups with NorthPoint Community membersWrite to incorporate Mental Health topics in the curriculum of General Wellness Group. Commit to doing health outreach once a week in NorthPoint’slobby. Develop surveying materials for group participants to assess effectiveness of program. Facilitate General Wellness sessions or assist Community Health Worker in running the sessions. Assess effectiveness of program and make necessary changes to improve the program

14. Red Wing CARE Clinic • Who we serve: Uninsured individuals who are unable ot afford healthcare and who are residents of Goodhue county and/or Lake City • Volunteer opportunities • Esp. spanish

15. Southside Medical and Dental Clinic • Who they serve: residents of Southern Minneapolis who do not have the means to get to a clinic or dental office to receive care.  With mobile units, the Southside Medical and Dental Clinic can provide care in a safe, non-threatening manner. Volunteer opportunities • Community Clinic Intern - Medical Mobile UnitWork with the Medical Mobile Unit to help fulfill the medical needs of southern Minneapolis neighborhoods while learning about how a community clinic operates.  This volunteer opportunity is a self-directed and intimate way to become involved in a Minneapolis neighborhood.   • Requires: Application, shadowing/training period, individual goal/project at the clinic, Dental and medical components • 4 Hour sessions
