  3. Advanced Development Program • Four 3 hour sessions • GP’s, Practice Nurses, Allied Health • Identify personal goals • Explore skills with colleagues • Try out skills in consultations between sessions

  4. Initial Outcomes – ADP: • Four complete ADP workshop series conducted successfully • High level of acceptance from participants • Improved quality of consultations

  5. “…..we tried agenda setting and it seemed to work really well. We were able to get her focused on the agenda for that appointment and then the agenda for the next few appointments as well.” • “I do cherish the moment when they have finally convinced themselves that they need to stop smoking or lose some weight without me telling them that. Pretty exciting time for consultations.” •  “I have changed care plans. I’m much more goal orientated now and when I do the reviews but they’re a lot less long winded now and it is easier to include the patient in setting some goals as opposed to just covering all aspects of a chronic condition. I probably narrow it down so it’s more useful for the patient.”

  7. Initial Outcomes – Service Re-Design: • 31 practices engaged • 33 Practice staff trained in Service Re-Design • 15 Clinical Staff received Interpersonal Skills Training • Identified changes occurring in service delivery

  8. “I was fortunate to have other staff members attend the Service Re-Design Workshop with me and it was wonderful to see how involved they were and how engaged they now are in the service re-design process.” • “Creating action plans at the workshop allowed us to identify our priorities on what we wanted to change and gave us great ideas on how to implement those changes” • “The detail in the reports was amazing and it was great to be able to compare our results to the national averages.”

Conclusions: • The number of health care providers who have the skills required for collaborative patient-centred care and the capacity of a portion of the existing workforce within the Division to support chronic disease self-management has been strengthened. • There are emerging changes to clinical practice with chronic disease patients particularly in the clinician’s use of newly learned skills and confidence in their own ability to partner with patients effectively.

  10. Conclusions continued: • General practices are motivated to invite patient feedback for the ongoing improvement of the delivery of their service and for improved patient health outcomes.

  11. Recommendations: • Clinicians continue to have access to the Advanced Development Program for skill development in order to be able to successfully partner with chronic disease patients in their journey of self management. • Ongoing support to practices for the improvement of service delivery to patients needs to be provided to embed a whole of practice system approach in the delivery of high quality care.

Recommendations cont.: • The use of an independent validated survey tool that provides benchmarking against peer practices be utilised for patient feedback. • Patient feedback should be required more frequently than current draft standards dictate to develop effective patient engagement and improved connectedness with practices in their development of patient focussed primary health care services.
