Immunization Quality initiative • 2 Goals: • Meet the immunization standards of State of Wisconsin. • Increase immunization rate of KCHC from 54% to target 85% by 2014.
Quality improvement projects Increase Newborn follow up rates Vaccinate all Pediatric patients seen at KCHC.
Vaccination Criteria Follow guidelines published by AAP ( American Academy of Pediatrics). Vaccinate Health checks/Preventive visits AND sick visits.
Newborn follow up Follow up conducted in accordance to Bright future guidelines of AAP. Follow up from 2 weeks of age to 24 months. Patients name noted and follow up call given 2 weeks prior to scheduled health check.
