School Counselor Responsible for scheduling all students and provides support as needed Provide Academic/Social/Emotional Counseling Testing Coordinator for GRAD/MCA Tests and works with new students Interprets credits for new students Meets with students and parents for credit evaluations Develops college/career resources for students Member of Child Study Team and Collaborative Problem Solving Facilitator of Student Support Team Student Council Advisor Organizes Graduation Ceremony Supports students in the development of CLP’s New Student Intakes New Student Orientation AVID Coordinator 504 Coordinator

School Social Worker Responsible for tracking truancy for students under 17 Meets with students as needed & for related services Crisis Management/Hennepin County Refers students to Child Protective Services as needed Works with Homeless and Highly Mobile Provides resources to students Assists with intakes Communicates with parents and provides support to families as needed SEL Instruction and Curriculum Collaborative Problem Solving Co-Leader Administering the Diagnostic Predictive Scale Staff consultation

School Psychologist Complete assessments and evaluations for eligible students Provide staff information on instructional and behavioral interventions for students Monitors implementation of interventions Social skills Communicate students needs with teachers Counsel students as needed Collaborates with special education team Assists with intakes

School Nurse Immunization status checks for NECA students and ELC Emergency care plans for applicable students Meets the needs of health concerns for students (limited health supplies: condoms, feminine hygiene) First responder for student and staff injuries Provides pregnancy tests IEP Assessments for students Supports the needs of students with medications

ALC Plus Case Coordinators Hennepin County Liaison Coordinates transportation for ALC Plus students Attendance calls daily to ALC Plus students Behavior support for ALC Plus students ALC Plus student intakes Family contact with ALC Plus students Community connections and resources for ALC Plus students Inform team of student’s outside factors (as appropriate) Get full picture of student and analyze data

Chemical Health Counselor Conduct chemical use pre-assessments for students with chemical health concerns Provide short-term CD counseling, referrals and follow-up for students and their families Assist students and their families to facilitate entry into treatment Consult and collaborate with Student Support Team and administrative teams regarding students with chemical concerns Coordinate program activities with community agencies to provide services to students and their families

  9. Marriage & Family Therapist Introduction will be made the first week in each classroom. Informal greetings to students in the hallways and cafeteria. Explain possible benefits of seeing a counselor to discuss personal matters/challenges/pursuits privately without having to leave the building. Available on Tuesday, Wednesday and Thursdays. Open door policy to staff and students for any type of consultation to assist toward a smoother school day. Collaborative approach taken at staff meetings, supplement and follow through with any mediation process, create a workable arrangement between staff and student, recruit staff to volunteer any ideas for a successful intervention because of course, we are a team. One can never communicate enough. If staff have any concerns about a student, please contact me at my office C-231/extension 7344. I look forward to working with all of you this new school term.

Referral Process If you need immediate assistance or it is a crisis situation please see the support person in your wing. • B-Wing: Allison or Maureen • C-Wing: Kristin or Emily & Paul • Suicide Concerns – consult with support staff immediately • Abuse & Neglect – all staff are mandated reporters

Referral Process If you have a student who may be experiencing: Academic Issues, Attendance, Lack of Motivation, Depression, Substance Abuse, Low Grades, Obscene Language/Gestures, Abuse, Mood Swings, Frequently Tired, Homelessness, Family Conflict, Emotional/Crying, Anxiety, Pregnant, Sexual Health, Medical Needs, Gang Involvement, Basic Needs (food, clothing, shelter), Trauma, Death/Grief, Suicide, Sexuality

  13. Referral Process • Once the form has been completed, please return to: • Kristin (C Wing Copy Room) mailbox • Allison’s (B Wing Copy Room) mailbox

  14. Referral Process Once a child has been referred to the Student Support Team… The team evaluates the students needs and then the most appropriate person meets with the student. Outcomes: Collaborative Problem Solving, Referral to Community Resources, Etc.
