4. Questionnaire:Hospital Demographic Information The ‘Hospital Demographic Information’ page now has a field to add the phone number of the hospital administrator A new question has been added regarding ‘Inpatient Surgical services.’

5. QuestionnaireOverall major section changes NEW! Tabs at the top of the screen allows the user to move easily from one RFI (Request for Information) to another NEW! Questions have Yes/No responses NEW! ‘Save Answer & Continue’ button New! ‘Do Not Wish to Participate’ option (when applicable)

6. QuestionnaireOverall major section changes Please note: the question tabs are now color coded • Question Not Answered Red - Question Answered Green Yellow - Question Incomplete (only for HAC section)

7. Upload Files: • New! Hospitals may now upload multiple files for each question • Each file is automatically saved once the ‘Upload’ button is clicked • To upload more than one document for the same measure, select the ‘Browse…’ button • New! There is now an ‘Upload Help’ button that allows the user to troubleshoot files that may be difficult to upload QuestionnaireOverall major section changes

8. ‘Comments’ Tab NEW! Hospitals can now view and respond to a comment from the PAV, per question directly from the Questionnaire page NEW! A colorcoded system of arrows indicates if comments listed were sent by the PAV (red arrow) or by the hospital (green arrow) The primary Q-HIP contact will still receive an e-mail notification that a comment has been made by the PAV NEW! The e-mail message received when comments are sent by PAV now reads: “Comment has been added by PAV in the XXX section.” QuestionnaireOverall major section changes

AdminOverall major section changes ‘Comments’ Cont’dAnother method of viewing and responding to comments is to use the ‘Review Hospital Data’ section of the web toolUnder the ‘Admin’ section, choose ‘Review Hospital Data’Then choose the section in which the email indicates comments are made (ex. IHI Improvement Map)Then click on the ‘comments’ tab to review your commentsThere’s a ‘Go To Question to Reply to Comments’ link which takes you back to the questionnaire page where you can reply to that comment

10. Documentation ExamplesEach question has a ‘Documentation Examples’ tab, found on the bottom of the page, with specific examples for what is needed for that measure

11. Evaluation CriteriaEach question has an ‘Evaluation Criteria’ tab, found on the bottom of the page for each specific question on the web tool

13. Admin ToolsOverall Major Changes All Evaluation Criteria NEW! Hospitals can now print all criteria for each section without the ‘Met/Not Met/Not Applicable/Not Evaluated’ grid The printed document is something that can be used as a clean worksheet by the hospital
