Methodology & Background • The Patient Navigation Center served 2,176 unique individuals during its first six months of operation • Patient Navigators assisted patients through 5,252 interactions, comprising phone calls, emails, texts, website live chats, and Skype calls. • The Patient Navigation Center caseload has grown 328% since opening in February 2016 • Health education inquiries have grown 315% compared to the same time period during the previous year (2015)

Stories from the Navigation Center • James • Uninsured, living in a state that did not expand Medicaid • Has severe psoriasis and suspected psoriatic arthritis • Not treating • Through working with his Navigator, we were able to: • Find him a new rheumatologist with very low visit fees for uninsured patients • Help him to get an official PsA diagnosis so he can get proper treatment • Help him get on a clinical trial for a biologic for continued and affordable care
Stories from the Navigation Center • Suresh • Graduate student from India, who has been accepted to start school in Texas beginning this summer • Suresh has severe psoriasis and uses a biologic to treat his condition • Through working with his Navigator, we were able to: • Help him gain insights into his new insurance coverage and to ensure he can access his biologic treatment • Explain insurance terminology and how to best utilize his coverage • Help him feel supported and able to excel in his studies without losing focus over his psoriasis • Suresh is very excited to be connected to the community of the National Psoriasis Foundation, and sincerely grateful for the support and education he has received
Stories from the Navigation Center • Kim • Called to request help with her costs with prescription topical, which she has been using for several years • She is uninsured and had been using her local pharmacy to get the treatment which charged her $140 for the prescription without any discounts • In working with her Navigator, we were able to: • Identify another pharmacy in her area that offers the prescription at a lower cost • Help her to enroll in the prescription assistance program for her treatment, which will pay up to $375 every month. • After visiting the pharmacy equipped with her new information and resources, Kim was able to get her prescription for $18 • Now her Patient Navigator is helping her finish her application for Medicaid
Stories from the Navigation Center • Davina • Uninsured, has severe psoriasis • Not treating or seeing a health care provider • In working with her Navigator, we were able to: • Finish her application for Medicaid, for which she was approved • Find a dermatologist who accepts Medicaid, treats psoriasis, and is enrolling new patients • Help her get enrolled in a biologic clinical trial • Davina is incredibly excited about her new treatment plan and the possibilities afforded by having insurance. She wants to give back by volunteering with the Foundation
Stories from the Navigation Center • Gloria • Has severe psoriasis • Phototherapy works well for her and her doctor had suggested a home phototherapy machine for treatment • Has Medicare coverage • In working with her Navigator, we were able to: • Discuss various home phototherapy equipment manufacturers • Connect her with the Insurance Specialists at the phototherapy equipment companies and provide the order forms to share with her doctor • Within two weeks, Gloria had a home phototherapy unit at her home, fully paid for by her insurance
Stories from the Navigation Center • Tom • Has severe psoriasis • Living abroad in Germany • Unable to find a dermatologist willing to prescribe a biologic (which he had been using for treatment while in the United States) • In working with his Navigator, we were able to: • Research options for U.S. treatment that would cover the cost of his medication • Connect him with a doctor at the University Hospital willing to prescribe systemic and biologic therapies • Now Tom is back on his treatment and seeing clearance
