1 Patient attends follow up appointment Following a recent angiogram, Mick returns to see his consultant, Dr Ataullah. The results show extensive triple vessel disease and Dr Ataullah recommends referral for a Coronary Artery Bypass. Tertiary Referral to Named Clinician
2 Consultant and patient discuss options Dr Ataullah explains that cardiac surgery is not carried out at this hospital, and that Mick will need to be referred elsewhere. He recommends a surgical colleague, Mr Appleby, in a regional centre. Tertiary Referral to Named Clinician
3 Consultant initiates the referral Mick is happy to accept the recommendation and Dr Ataullah arranges for his clinic staff to initiate the referral to Mr Appleby’s service. Tertiary Referral to Named Clinician
4 Administrator makes referral Dr Ataullah’s administrator uses the NHS e-Referral Service to make the referral on his behalf and gives Mick the booking instructions which contain links to further information about the hospital - including a map. Tertiary Referral to Named Clinician
5 Administrator submits referral information Dr Ataullah’s administrator ensures that all Mick’s relevant details and results are downloaded from the hospital Electronic Patient Record and added to his referral as electronic attachments. Tertiary Referral to Named Clinician
