e-Health based chronic disease management: the e-trikala telehealth service Individual citizens are equipped with light-weight handheld devices and record their vital signs which are then transferred (via the telecare center) to the General Hospital of Trikala over PSTN or GPRS for review and feedback.

Telecare Services of the Municipality Instead of public procurement of a telecare platform existing in the market, a joint partnership with research institutions, end user organizations, companies was selected.

EMR and Telehealth Service for the ROMA community of the Municipality of Trikala
Challenges and actions • EMRs of HIS of the Regional Health Authorities in Greece have recently been developed. • Problems still exist in the interoperability of the EMRs between the RHA • Portable EMR is an advance service not only on national but also on european level • A study commissioned by the Prefecture of Thessaly, before the pilot implementation. • Partnership with the Regional Health Authority of Thessaly
Progress so far • 11 Municipalities of Central Greece, representing more than 1.000.000 citizens, have recently joint forces together and expand the “best practice” of the Municipality of Trikala • Via the Operational Programme 'Digital Convergence' of the European Territorial Cooperation Programmes, a regional telehealth network will be funded. • The experience of procurement procedures so far, can benefit the regional partners • Municipality of Trikala became a competence center on regional level • Incubation of innovative practices and projects
