2. DISCREPANCY CODES: WHAT ARE THEY? • Codes are used by all branch services to identify specific discrepancy. • The codes are controlled by DoD and determined by each branch service to be a “Fatal” or “Tested”.. • FATAL code: Non Testable • TESTED code: Testable, but discrepant. • Each code has its own unique identifier: • Bottle discrepancies begin with “B” • Form discrepancies begin with “F” or “G” • Label discrepancies begin with “L” • Packaging discrepancies begin with “P” • Specimen discrepancy begin with “S” (internal)

MOST COMMON FATAL CODES • BC – Bottle leaked in shipment, quantity not sufficient to test • BU – Bottle empty • BY – Bottle discrepancy – NOT TESTED • GG – Form listed specimen, no bottle received • LY – Label discrepancy – NOT TESTED (missing) • PY – Package discrepancy – NOT TESTED (Damaged) • SA – Specimen appears to be adulterated – NOT TESTED • SC – Specimen quantity not sufficient to test

MOST COMMON TESTABLE CODES • BD – Bottle seal broken • BF – Bottle with two seals, no explanation • BU – Bottle empty • FA – Form, UIC or Base/area code discrepant/differs from bottle • FL – Form not received • FN – Form chain of discrepancy discrepant (blks 12a-d) • FP – Form did not list specimen, bottle received • FR – Form on two pieces of paper – no linking identifiers

5. MOST COMMON TESTABLE CODES • GP – service member’s name/signature on forms • GZ – Form discrepancy – TESTED • LJ – Label: Member initial discrepant • LL – Label: Collector or observer’s initials discrepant • LN – Label: SSN does not match the form • PA – Package: No seal • PB – Package: Broken seal • PD – Package: Missing signature/date • PY – Package: Tested • SB – Specimen appears to be adulterated* • SE – Specimen volume < 30 mL

6. REDUCING DISCREPANCIES • Make sure you are following the procedures outlined in OPNAVINST 5350.4D, ENCL 2, Appendix B. They are also available on the NADAP website, NDSP page, titled Packaging and Shipping information. • Double check everything prior to sealing up the samples. It’s a good idea to use a second set of eyes in this process. • If you don’t have certain packing or packaging material and want to use something different, contact the office of NADAP or servicing lab for guidance.

7. REDUCING DISCREPANCIES • Make sure you manage your discrepancies like you would a positive result. Don’t ignore them, act on them. • Conduct training with assistant UPCs. • Contact your ADCO (Alcohol and Drug Control Officer) or NADAP if you need assistance.
