Improvement Calculator • Calculates baseline rate • Pre populates targets and cost per HAC • Graphs monthly rates • Calculates YTD Improvement • # of lives impacted • Costs avoided • Gap to target
Click on L4 Enter # of 30 day all cause readmissions for baseline period
Click on M4 Enter # of discharges for the baseline period
As monthly data is entered, the calculator will calculate your improvement
Calculator will estimate the # of readmissions to prevent to reach the goal next month and graph your results.
Harm Across the Board Calculation • Once data is entered for applicable topics, harm across the board will be calculated • Harm across the board sums all harms divided by patient discharges • ADE • Falls with Injury • Stage III/IV Pressure Ulcers • CAUTI • CLABSI • VAP • SSI • EED • VTE
Discharges must be manually entered. Tip: Cut and paste from Readmission tab Harm Across the board includes all harms except Readmissions
Harm Across the Board • Hospital templates are due the week of June 3 • HRET will select top stories to be presented at July 24-25 Quality Meeting • Participants will vote on top stories • Top stories will be on stage with Don Berwick at the meeting
TEMPLATE GUIDE • Treat harms as events that can be summed • Focus on harms (outcomes) rather then preventive measures (process) • Special conditions can be considered a harm (e.g., EED, Readmits, …) • Produce an overall harm trend for the hospital (**Delete this slide when content of presentation is complete)
