FMC Fall From Height 6 September 2010. Events.

Events • IP entered a “closed” scaffold structure. This was a scaffold around a test stack up of equipment. The IP was employed by the subcontractor hired to perform jacking operations on the test stack. The scaffold had been modified to avoid collisions between the structure and the scaffold. • IP climbed to 3.7 or 5.7 m level. • IP fell to the ground via a pipe at the 3.2 m level, landing on the ground head first. • IP’s helmet was found on 3.7m level. • The IP was knocked unconscious and sustained a serious head injury. • The IP was found by one of his colleagues • There were no witnesses the IP has not regained consciousness.
Events • Scafftag was turned. Normally to close a scaffold tag is removed. • SJA & PTW did not cover the inspection task being undertaken by the IP at the time of the incident. • IP had attended the TBT for the main task. • IP worked for a subcontractor to Grenland Group, who were FMC’s main contractor for the work. • IP is likely to have been aware that scaffold had been modified.

Short Term Actions Arising • Management organisation & responsibilities clarified • Work permit system applied to all work including routine work • Work permits reviewed at morning meeting. • Work permit validity restricted to 24 hours. • Weekly HSE awareness meetings with all on site • Increased HSE resource presence at site from 1 to 2. Also 100% presence during week‐ends. • Implementation of new type of safety helmets of climbing type for all personnel working on site
