  5. Person Support Plan • Case Study • Mary has an intellectual disability and she requires a individual support plan to assist her develop social skills.

Support plan • Documenting the support plan • Maintaining regular assessments on performance of tasks which should include knowledge of skills. • Staff and individual have access to the plan and maintain up to data in accordance with their role • Maintain up to date notes regarding achievements and any challenges or difficulties experienced.

Support Plan • Document any information pertaining to reviewing goals and strategies • Involve the client in updating the documentation in the support plan this should include self evaluation • Ensure the documentation support plan is used at review sessions. Parents, guardians, carers can be included where appropriate. • Refer to the support plan for regular reflection and feedback

  10. Skills development • Focus on clients strengths and abilities • Monitor progress on achieving short and long term goals • Skill Development include: Social skills, life skills, vocational skills and employability skills.

Class exercise Refer to the case study In groups prioritise the needs of the client. • List all the needs of the client. • Arrange these in order referring to Maslow Hierarchy of needs. • Consider the person as a whole. • Place the needs in referring to Maslow Hierarchy of needs and the persons needs. • It is important these needs are placed in order of priority.
