Through reflective learning and engagement principles, the tool helps create a safer work environment by changing behavior and ensuring incidents are reported and supervised properly. The Objectives Simple Tool Reporting Culture Easy access and user-friendly reporting tool ` Employees are encouraged to report near misses without fear of disciplinary action or loss of job. Easy Tracking Learning Enables PDO and contractor companies to pro-actively eliminate and/or control hazards before a tragic or costly incident occurs. Reporting and tracking near misses can provide valuable information as to where the gaps in the HSE management system exist.
What is a Near Miss? It is an unplanned event that has the potential to cause harm -but does not- to human , environmental or equipment.

Principles of TBT Reflective Learning MSE3 team facilitate the reflective learning tool box talk (TBT) training programme. Taking key people from PDO and its contractors through reflective learning techniques which they will then pass to supervisors in the field in how to deliver a high quality TBT and ensure engagement with their teams in “Learning from Incidents”. By implementing this “real learning” approach we will actively engage their hearts and minds leading to a change in behaviour which will create a safer future for us all. How ? By Employee engagement

Ensure all incidents are reported • Ensure Supervisor’s are supervising • Ensure the task is done using hands off techniques where possible • Mechanical lifting devices should be used when required A floorman, with the help of two colleagues attempted to unscrew and remove the sucker rod (SR) table in order to adjust the SR wiper rubber. As they were removing the table, it slipped from the hands of one worker tilting to one side trapping the floorman’s left hand against one of the stripper screws resulting in a fracture of his left hand little finger. Use hands-off where possible كان عامل الارضيه و بمساعدة اثنين من زملائه يقومون بفك وإزالة (Sucker Rod Table) من أجل ضبط الممسحة المطاطية. عندما قاموا بإزالة الطاولة، انزلقت من يدي أحدهم لتميل إلى جانب الآخر لترص اليد اليسرى لعامل الأرضية ضد احد المسامير مما أدى إلى كسر في الاصبع الصغيره لليد اليسرى. لا تستخدام اليدين حيثما أمكن • تأكد أنه تم الإبلاغ عن جميع الحوادث • تأكد أن المسؤول يشرف على العمل • تجنب استخدام الأيدي حيثما كان ذلك ممكنا • يجب استخدام أجهزة الرفع الميكانيكية عند الحاجة
