Diabetes Medical Management Plan (DMMP) • Basis for all school-based diabetes care plans • Developed by: -student’s personal health care team -parent/guardian • Signed by a physician or Advanced Practice Registered Nurse (APRN) • Individualized to student • Coordinated collaboratively by the school diabetes team: -school nurse -health coordinator -school administrator -other school personnel (Delegatees) ` -parent/guardian -the student KBN 2014
