What’s happened since last year • Last year we advised that we were moving to a single social care data collection via Source. A lot has happened since then: • Data collection has been reviewed. • Consultation took place with data providers and data users. • Final dataset agreed and definitions and guidance document updated to reflect new data collection. • Data Sharing Agreement was updated to reflect new data collection and changes required for GDPR. • Data providers have worked hard on producing updated data and submissions have been received.
Content of Source Social Care dataset • Self Directed Support • Home Care/Reablement • Telecare/alarms • Care Homes • Day care • Meals • ioRN • Supported by Demographics (incl identifiers)
Provisional Plan for data submission • 2017/18 Part-year data • Available from late 2018 • 2018/19 Full-year data • Requested in two 6 month blocks • 2019/20 + Full-year data • Updated quarterly
Health and Care Pathways Linked patient level data allows pathway analyses to take place.
Health and Care Pathways Linked person level data allows pathway analyses to take place. Person level Social Care data allows broader understanding of what is happening.
THANK YOU… HSCPs’ submission of data is a crucial first step to establishing the revised, single, nationally-collected Source Social Care dataset. HSCPs have worked hard to submit this data, and to collaborate with ISD to ensure that it is checked, complete and ready for analysis.

What we know from Social Care data - Demographics- Age group & Deprivation
Care Home Residents - Demographics- Age group & Gender
Home care clients Characteristics - Living alone & Average Care home Hours
Care homes: Which care homes have highest Percentage of A&E attendances that result in admission to hospital?
Care home residents: main reason for emergency admission to hospital
Home care clients Linked with hospital admissions to find out about long term conditions
Social care clients Linked with records that have a SPARRA score (risk of admission/readmission to hospital) High Risk
Social care clients: Locality Linking falls data with clients in receipt of a Social Care package at time of fall
Potential linkages of Social Care Data to Health Data - Future • Individual person level to National level. • Scope to link Hospital admissions, Community, Mental Health, Prescribing, Outpatients and Deaths. • Resource and service consumption e.g. Cross sectional analysis of social care services. • Predictive modelling / Projections of activity of linked health and social care data to demonstrate the flow of services. • Social care packages pre or post hospital admissions • Patient outcomes on populations and specific cohorts.
