This directive outlines the filing procedures for E-Rate reimbursement for all State Master Contracts starting with FY2013-2014. DivTel will manage the procurement, billing, and USAC invoices, reducing the burden on customers. Customers maintain choice of invoicing modes and will not have to submit AT&T GRIDS. DivTel will ensure accurate applications and compliance with USAC guidelines.

USAC Directive • USAC requires DivTel to file for E-Rate reimbursement for all State Master Contracts starting with FY2013-2014 • Decision due to fact that DivTel pays service provider invoices – i.e. is Billed Entity • The change applies to all DMS-billed E-Rate Eligible SUNCOM Master Contracts • Customer must file Form 471 for DMS services directly billed by the vendor (e.g., Mobile Communications Services)

Benefits to You • DivTel manages all procurement and competitive bidding for state master contracts • DivTel will be responsible for filing applications, billing and USAC invoices • Customers will not have to comply with USAC State Replacement Contract procedures

DivTel, rather than SUNCOM customers, will be responsible for errors or omissions that impact funding.

Competitive Bidding: ‘Mini Bids’ • What is a mini bid? It is a process used by applicants to evaluate all eligible state master contracts and demonstrate why the selected service provider it chooses is the most cost-effective solution. • When is a mini bid required? A mini bid is required for contracts where a state files a Form 470 and signs state master contracts with more than one provider. • Applies to the following SUNCOM contracts: • Mobile Communications Services (MCS) # DMS 1011-008(A or B, depending on vendor) • Telephone Equipment & Services (STEPS) # 730-000-09-1 • Wiring, Internal (TIPS) # DMS 1213-001(A-N, depending on vendor)

DivTel Will Need From You: • Letter of Agency – Sign, date, notarize, and return to E-Rate Team • If ordering P2, you will need a Technology Plan • If you already have a certified Tech Plan: • Provide your DOE letter of Tech Plan Certification • If you don’t have a certified Tech Plan: • Notify E-rate Team of Plan status • You must provide your DOE Letter of Tech Plan Certification beforeForm 486 (Receipt of Service Confirmation) can be filed • Inform E-Rate Team if you are conducting your own procurement (filing own Form 470) and considering a DMS State Master Contract as a bid response
DivTel Will Need From You: • Services, quantities, and other information included on Block 5 • Copy of Form 479 (Children’s Internet Protection Act Compliance Certification) if ordering Internet Access services • Contact information for person(s) responsible for E-Rate • Any supporting documentation requested by E-Rate Team
