3. Intra Aortic Balloon Pump (IABP)
CONSOLE
- drives the balloon to inflate and deflate
- contains helium in a small tank which inflates the balloon
- timing of the movements is controlled in careful synch with the rate and rhythm of the heart
7. INDICATIONS The following situations may benefit from this device.
Cardiogenic shock when used alone as treatment for myocardial infarction 9-22% survive the first year.
Reversible intracardial mechanical defects complicating infarction, i.e. acute mitral regurgitation and septal perforation.
Unstable angina pectoris benefits from counterpulsation.
Post cardiothoracic surgery most common and useful is the use of counterpulsation in weaning patients from cardiopulmonary bypass after continued perioperative injury to myocardial tissue.
Preoperative use has been suggested for high-risk patients such as those with unstable angina with stenosis greater than 70% of main coronary artery, in ventricaular dysfunction with an ejection fraction less than 35%.
Bridge to heart transplant for those patients with left ventricular failure.
Percutaneous coronary angioplasty

12. Percutaneous transluminal coronary angioplasty (PTCA)
An invasive, nonsurgical technique in which one or more arteries is (are) dilated with a balloon catheter to open the vessel lumen and improve arterial blood flow,
PTCA may be used for clients with an evolving myocardial infarction (MI), alone or in combination with medications to achieve reperfusion,
The client can experience reocclusion after the procedure; thus, the procedure may need to be repeated,
Complications can include arterial dissection or rupture, immobilization of plaque fragments, spasm, and acute MI.

16. Preparing Patient for a Procedure Maintain NPO status after midnight.
Obtain informed consent, allergy assessment to iodine/shellfish allergy (contrast media).
Prepare the groin area with antiseptic soap and shave per institutional procedure and as prescribed.
Assess baseline vital signs and peripheral pulses.
Instruct the client that chest pain may occur during balloon inflation and to report it if it does occur
17. Cardiac Angiograms
19. MEDICATIONS Plavix Clopidogrel
Indications
- Reduction of atherosclerotic events (myocardial infarction, stroke, vascular death) in patients at risk for such events (recent myocardial infarction, stroke, or peripheral vascular disease.
Integrilin
Patients who are to be managed medically and those undergoing PTCA
Acetylcysteine
- Oral acetylcysteine is used for the prevention of radiocontrast-induced nephropathy (a form of acete renal failure). Some studies show that prior administration of acetylcysteine markedly decreases (90%) radiocontrast nephropathy
