Who are LCHS • Provides around 140 health and community support programs • Employs 428 staff • Provides services in clients homes, at LCHS sites or to other health and community services across Gippsland

What is Gateway? • The Gateway program encompasses five areas: • Commonwealth Carelink • Service Access (central intake) • Emergency Relief • Aged Care Assessment Service (ACAS) • After hours on-call service
Gateway • Gateway aims to provide clients: • More efficient and effective access to services • Improved service coordination resulting in improved client outcomes
Service Access team • Understand and provide information about more than 140 different LCHS programs • Process approximately 18,000 referrals every year • Understand and provide information about a variety of services external to LCHS
Incoming referrals • Referrals to any LCHS program can be made by: • S2S electronic referral system • Email • Fax • Phone • Walk in
Initial Needs Identification (INI) • Service Access Officers (SAO) identify the needs of the client through an Initial Needs Identification (INI) process using the Service Coordination Tool Template (SCTT) • On completion of the INI process and prioritisation, client consent is obtained and referrals sent to the appropriate services
