What is PGPPP? • Australian Government Initiative that provides Junior Doctors with an opportunity to experience a supervised General Practice placement as an accredited 10-11 week hospital rotation • Thinking of GP training? PGPPP will expose you to the daily life and challenges of General Practice in a supported team environment • Thinking of other specialties? PGPPP will provide you with a unique opportunity see how community care integrates with that in hospitals and how GPs and other specialists can work together
Who Manages The Program? • The PGPPP program is administered by the Australian General Practice Training (AGPT) program. • The Regional Training Providers (RTPs) that deliver the AGPT program, deliver the PGPPP program • This means PGPPP placements occur in accredited teaching practices who are experienced and enthusiastic about teaching
Who Manages The Program? • There are 17 ‘RTPs’ in Australia, with 6 in NSW • General Practice Training – Valley to Coast manages this program in part of your network. We cover the beautiful Hunter, Central Coast and Manning regions.
GP Synergy RTP… Who are we? GP Synergy is the largest provider of vocational General Practice education and training in NSW, delivering prevocational and vocational training across central and southwest Sydney, and the New England/Northwest region of NSW. Where can you do a placement? • Hospital Network: 12 – Newcastle, Maitland, Manning (Taree), Tamworth and Armidale hospitals • Location: Newcastle – Blackbutt Doctors Newcastle Mental Health Service (PGY1+) Tamworth - Northwest Health 1-2 more in New England and Newcastle in 2013 • All PGPPP placements are conducted in accredited RTP training practices, with experienced GP Supervisors
Why do a PGPPP post? • Clinical autonomy – one on one direct care and continuity of patients • Excellent educational experience with individual supervision and mentoring by experienced and committed GP Supervisors who provide planned, structured teaching to support the clinical experience • Own patient load to build relationships and experience continuity of care • Work with Allied Health Professionals • Exposure to team care involving public and private providers
Why do a PGPPP post? • Overall understanding of the Australian Health Care System • Interaction with staff in General Practice, learning in teams of clinical practitioners • Experience and observe a professional small business in operation • Benefit from one on one mentoring • To be a better specialist
Who can/should do a post? • All Junior doctors who have not yet enrolled in a speciality training program should consider a PGPPP placement, even those not necessarily considering a career in general practice:- • Junior Doctors who are not sure of their career plans • Junior Doctors not considering a career in General Practice and who have decided on another speciality career choice • Junior Doctors already considering a career in General Practice
Supervision • Supervisor on site 100% of your placement • Amount of Supervision varies as you progress through the stages • Stage 1: Observed Supervision • Stage 2: Waive Supervision (reviewed at end of each consultation) • Stage 3: Reviewed Supervision (reviewed at end of each session) • Stage 4: Independent Supervision • Movement from Stage 1-3 at the discretion of the Supervisor • Movement onto Stage 4 requires a Clinical Teaching Visit by an RTP Medical Educator
Teaching • Minimum of 1 hour in practice structured teaching/week • Significant practice informal teaching/week in a variety of forms including case discussions, corridor consultations, shadowing, practical skills sessions, tutorials, case studies, video consultations and review, amongst others. • Opportunity to attend RTP half day/full day release Registrar workshops
Orientation • Workshop on first day of term with Valley to Coast in Newcastle • On commencement at your General Practice • Equips you with some of the essential knowledge to help get you started in a general practice setting
JMO Support • Dedicated PGPPP Program Officers at each RTP help manage and coordinate PGPPP Terms • Dedicated PGPPP DPETs oversee the PGPPP educational program and act as a liaison point for participants and hospital DPETs • Medical Educators available Monday - Friday
Typical PGPPP Term • Orientation with RTP on first day of term • Week 1 – Commence at the Practice(Induction/Observation) • Week 1 – Contact with DPET at the end of the week • Week 2 – Contact with program officer • Week 4 or 5 – Teaching Visit by DPET • Week 5 – HETI mid term assessment • At least 2 full day GP Registrar Workshops with Valley to Coast • Week 10 – HETI end term assessment • Last week of term – DPET wrap up session (debriefing)
PGPPP Q’s & A’s • Who can apply for a PGPPP term? • PGY2 + JMOs • How long is a PGPPP placement? • 10 -11 weeks long – consistent with hospital rotations • What hours will I be working? • 38 – 42 hours per week (time accrued over 38 hours) • How will I be paid? • You remain an employee of your hospital and will continue to be paid and accrue leave entitlements in the usual way
PGPPP Q’s & A’s • Can I take leave during the Term? • Taking leave during a PGPPP rotation is discouraged but will be considered in exceptional circumstances • Will my consultations be supervised? • You will be required to consult your Supervisor about the management of patients • As you move through the Supervision stages the review/discussion of patients care and management will be negotiated with between you and the Supervisor • You and your Supervisor share responsibility for individual patients
PGPPP Q’s & A’s • Provider/Prescriber number eligibility? • Yes, PGY2+ are eligible for a Provider/Prescriber number (The RTP sorts this out) • Are my consultations billed to Medicare? • Consultations generated by PGY2+ are billed to Medicare • How many patients will I see? • Depends on your Supervisor’s assessment of your capabilities • No more than 2 - 4 per hour during the term
