The aim of the strategy • To facilitate the development of appropriate new extra care housing schemes • To facilitate and in many cases fund support and unplanned care services in extra care schemes • To work in partnership with housing authorities, housing associations and the private sector to achieve these aims.

Demography • Number of older people is projected to increase significantly: • In the UK, the number of people aged over 65 is projected to rise from 10.1 million to 16.7 million over the next 25 years • In 2008, there were 1.3 million people in the UK aged 85 and over. This number is projected to more than double to 3.3 million by 2033 • People aged 85 and over are more likely to have care and support needs, reflecting the greater frailty and higher prevalence of dementia in this age group

  5. Demography Population aged 85+ by District The number of people aged 85 years and over is projected to increase by 103% between 2011 and 2031 across Dorset.

  6. Demography Dorset CC has the highest percentage of older people of the 16 South West counties and unitaries, and the lowest percentage of working age people. Christchurch has the highest percentage (29.5%) of people aged over 65 of any district in England.

Extra Care is an ideal option For older people who: find it hard to cope in their current home are living in residential care but find the level of care and support is not needed are in hospital and need to return to accommodation with support and care available 24 hours day wish to maintain their independence and their own front door

A definition of Extra Care Self contained accommodation incorporating design features to facilitate independence and safety 24 hour care and domestic support available in the individuals own home if required Meals available An alarm system Extensive communal facilities Staff offices and facilities Specialist equipment to help meet needs of more frail or disabled residents such as assisted bathing Social activities on site and/ or arranged.

Distinguishing extra care Key features that distinguish extra care from residential care homes are: • Self contained accommodation not simply a room (including en-suite rooms) • Provision of care can be separated from provision of housing • Care can be easily be based and delivered on an individual basis • Occupiers can be assured tenants or owners with degree of security not licensees.

  10. Culture At the heart of the development is its culture or ethos. This is evidenced in values and practices that promote independence. This would be apparent in, for example: Work to maintain service users independence Philosophy of ‘working with’ residents with residents participating in running scheme or activities Wide range of social activities and continuing links with wider community Relatives’ active participation in provision of support and care encouraged not discouraged Barrier free environments and design which is enabling

An evaluation of extra care housing schemes East Sussex County Council The most significant findings include the following: When assessing where residents in the schemes would live if they were not living in extra care housing, 63% were judged as needing residential/EMI/nursing care; The enabling design and accessible environment of extra care housing supported self care and informal family care, thus increasing independence; The evaluation outcomes strongly support the preventative nature of extra care housing; The importance of the on-site restaurant was emphasised, not only for nutritional and health impacts, but also as a social hub and springboard for social activities.

An evaluation of extra care housing schemes East Sussex County Council The financial impact of the findings was considerable, with the evaluation indicating that the cost of extra care housing was on average half the gross cost of the alternative placements. Capital invested in the schemes by the Council was recovered, depending on the scheme and size of contribution, between 1.5 and 3.3 years.

  14. Trailway Court It is apparent that health care costs dropped significantly for both groups whilst social care costs rose. In both cases unmet need was addressed. However, social care costs rose more for the control group after 4 people went into residential care. The data suggests that: It costs money to do nothing Great outcomes can be achieved in Extra Care for a cost similar to low level domiciliary care Extra care can be a viable alternative to high cost residential care right up to end of life

Extra Care Housing Bure House, Westhaven House and Foylebank Court • New combined care and support contracts went live in July or September 2013 – 3 to 5 years • Settling in programme (Reablement) and Health & Wellbeing Programme – support + unplanned care • Staff – Scheme Supervisor (18.5 hours), Activity Coordinator (18.5 hours) and 24 hour on site Dom Care worker • Some packages by on site staff and discounted from contract price

SHOP Strategic Housing for Older People Analysis Tool (SHOP) • This tool uses data generated by Elderly Accommodation Council's national records • SHOP can interpret local information at a unitary/county level or for individual districts • This can help shape decisions on the how, what and where to develop specialist housing for older people

  18. Demand The SHOP toolkit suggests per thousand people over 75 years there should be: 20 ‘enhanced’ sheltered housing properties 25 extra care properties Have assumed 50:50 split between owner occupied and social housing
