Reasons not to issue OTR Statement • Application for OTR evaluation and supporting documents are either incorrect or untimely submitted • The Company refuses to undergo OTR evaluation procedure • The equipment capabilities do not comply with the established requirements • Insufficient personnel qualifications • The applicant is not ready to perform the types of work listed in the application • Supporting documents submitted for initial review are either forged or do not comply with the established requirements • Mandatory permits/approvals are missing (certificates of self-regulating organizations, licenses, etc.)
