External audit findings • Dealing with multiple findings that could (or could not) constitute PISAs in hours to days • Consult DOE w/o delay; agree on approach with expeditious schedule; prioritize; disposition each finding • Imminent hazard – address as PISA without delay
Design basis reconstitution(similar to external audit situation) • DBR efforts qualify as DSA upgrades • USQ doesn’t apply for new requirements or different analysis methods (prob/conseq) • USQ applies when safety basis is not bounding or otherwise inadequate • Project should have a process for prompt sorting of questions and issues as: • Normal part of reconstitution • PISAs
Control of hazards during installation of modifications • Worker hazards dealt with under normal work control procedures • USQ invoked if and when facility safety basis may be affected
EFCOG Issues • Screening • Simple go/no go decision • Is vs As (described in the DSA) • Evaluation of the safety of the situation (PISA) and JCO • Discernable increase • Major modifications • Transportation
Operability Determinations • Non conforming SSC (loss of quality or functional capability) • PISA – place the facility in a safe condition – shutdown? • Forward-looking determination • Immediate: best available info and operational restrictions, if imposed • Final documented: analysis, tests, operating experience, engr. Judgment, availability of other equipment, conservatisms and margins, etc. • Final determination – part of the evaluation of the safety of the situation
