  3. From the OSIIS Landing page, select “Inventory” from the Toolbar OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.

  4. Select “Vaccine Request” from the Inventory drop-down OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.

  5. This takes you to the Site Inventory – Vaccine Requests screen Vaccine requests already submitted for your site, but not yet processed are listed here You may edit a pending vaccine request by clicking here OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.

  6. Enter the information you wish to change on the Edit Vaccine Request Screen Click Save OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.

  7. The Add Vaccine Request screen will appear Any changes made will be reflected after saving To create a new vaccine request, click on “Add Vaccine Request” OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.

  8. Complete all of the fields on this screen Click Save OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.

  9. Your new vaccine request will appear after saving OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES.
