WHY Person centredplanning? • Persons with disabilities have the RIGHT to person centredsupportthatenablesthem to live in the community. • The European Union and most Member States ratified the United NationsConvention on the RightsofPersons with Disabilities. In this way, they committed to making surethatpersons with disabilities can get the support they need to live independently and take part in the community. • A high proportionofdisabilityservices are still provided on an institutionalbasis, in carehomeswhich are isolatedfrom the community. • The transitionfrominstitutional to community-basedcarerequiresorganisations and carers to re-think the way supportservices for persons with disabilities are designed, organised and delivered.
