Transformation of Community Diabetes Services in Nottingham City

Key Features of the service: • Single point of access • Clinical triage • Structured education for type 1 & 2 diabetes • Care planning (Year of Care) • GP practice support – including joint clinics • Multi-disciplinary specialist clinics • Continuous blood glucose monitoring • Access to psychological support • Advice, support and training for healthcare professionals
The Team: • Diabetes Specialist Nurses • Consultant Diabetologists • Diabetes Specialist Dietitians • Specialist Diabetes Educators • Access to psychological expertise • Administrative co-ordinators • Clinical support workers
Jane 45 years old and has Type 2 diabetes • Initial care through GP surgery • Refer to JUGGLE – Jane can self refer • When you need more support – e.g. on two or three oral therapies and not sure what to do next. • If Practice nurse does joint clinics – can book into one of the joint clinics. • If Practice nurse does not do joint clinics – refer to hub and they will be triaged to your DSN clinic
Other things to consider • Dietetic referral • Healthy Change (or whatever it is going to be) • Psychological support • Repeat Juggle attendance • Fit4Life • Just make a phone call and ask or discuss with you DSN when they come to your practice
Jane is 30 and has Type 1 diabetes • Refer to hub unless – acutely unwell/new diagnosis/renal problems/ foot problems/ pregnant • If currently attending NUH you can have a conversation with the patient about changing to the community service. • Confusion arises when seeing NUH and community
Planning Pregnancy for diabetes • Anyone of child bearing age who is sexually active should be asked if using contraception. • If not using contraception then they are planning a pregnancy. • Pre pregnancy planning slot at both hospitals for advice. • HbA1c check / annual review check / folic acid 5mg/drug check • Tell the hospital as soon as know pregnant – happy to take referral from anyone but need background information in a letter.
