1. Federal Financial Report (SF-425) Additional Guidance for ASHA Grantees
2. Revised Quarterly Progress Report (QPR) Requirements The following QPR submission requirements are effective for the quarter ending 12/31/2009.
QPRs will include,
New SF-425
Quarterly Narrative Time-Line and Progress Report (NTL) on each major activity
and when applicable the current MS-Report on Commodities (ROC)
QPRs will not include,
The SF-269 and the PSC 272 (Letter of Credit Grantees)
QPRs are due to this office 30 days after the end of the calendar quarter.
If the grant includes federally funded construction, a copy of the QPR will also be sent to the ASHA engineer.
3. Guidance on the SF-425 for ASHA ASHA requires the submission of separate SF-425 forms for each grant
The SF-425 can be downloaded from the http://www.whitehouse.gov/omb/grants_forms/ web site and filled out by the grantee
If the grant is funded with a letter of credit, lines 1 – 9, 10d – 10o (excluding 10a – 10c), and lines 12 – 13 should be completed for the grant
If the grant is not funded with a letter of credit, Lines 1 – 10, and 12 - 13 should be completed
Line 11, Indirect expense - is not applicable to ASHA grants
4. Guidance on the SF-425 for ASHA Cont’d SF-425A form is not to be submitted to ASHA
The FINAL SF-425 is to be included in the grantee’s Final Project Report (FPR) which is due to this office 90 days after the date of the completion of the grant. The FPR should also include:
Three 8x10 color prints or digital photos, or a compact disc (CD) with photos of the project/equipment,
The Report on Commodities (ROC) marked “FINAL”, if appropriate, and
The Final Project Report and Evaluation (FPR) that includes an updated Performance Management Plan

5. Significant Differences Between theSF-425 and the SF-269 The information on the SF-425 is similar to that on the SF-269 except that the SF-425 does not report costs by categories (i.e. construction services and commodities). The SF-425 reports total costs.
Grantees are required to internally account for costs by cost category and are subject to:
ASHA’s right to request additional information when questions arise during ASHA’s administrative desk reviews,
OMB Circular A-133 audits, and
Site visits
