  2. El embarazo no se puede iniciar o mantener sin la progesterona. Para ejercer su acción la progesterona tiene que fijarse a receptores específicos. Han sido sintetizados mas de 400 compuestos con posible actividad antiprogesterona, pero hasta el momento la más completa es la Mifepristona (RU486)

  3. La primera publicación con datos de laboratorio y clínicos sobre RU486 fue hecha por Herrman en 1982 Herrmann W, Wyss R, Riondel A, et al. Effet d'un steroide anti-progesterone chez la femme: interruption du cycle menstruel et de la grossesse au debut.. C R Acad Sci III 1982;294:933-8 .

  5. RU486 es un 19-noresteroide con alta afinidad por los receptores de la progesterona. También tiene fuerte actividad antiglucocorticoide. Baulieu EE. Contragestion and other clinical applications of RU 486, an antiprogesterone at the receptor. Science. 1989;245:1351-7 .

  7. LA MIFEPRISTONA: • Es insoluble en agua. • Se disuelve muy rápidamente en el medio gástrico. • Se presenta en tabletas de 200 mg. • Se mantiene estable después de 3 años a temperatura ambiente. “Mifeprex Labeling at http://www.fda/eder/fai/lobel/2000/20687 ebl.htm ”

  8. USOS DE LA MIFEPRISTONA: • Uso contínuo en contracepción. • Inducción de la falta de menstruación. • Terminación del embarazo en el 1er trimestre. • Terminación del embarazo en el 2do trimestre. • Maduración del cuello uterino. • Inducción del parto.

USOS DE LA MIFEPRISTONA: • Tratamiento de la endometriosis. • Tratamiento del leiomioma uterino. • Tratamiento del meningioma. • Tratamiento del cáncer de mama. • Tratamiento del Síndrome de Cushing. “Clinical application of Mifepristone (RU486) and other antiprogestins. ACCION DE LA MIFEPRISTONA (RU486) SOBRE EL APARATO REPRODUCTIVO • Cuando se administra al inicio de la fase folicular, inhibe la ovulación y puede claramente ser contraceptiva. • Bloquea la ovulación y por tanto la producción de progesterona directamente a nivel del ovario. • Aumenta la sensibilidad del miometrio a la oxitocina y las prostaglandinas.

  11. ACCION DE LA MIFEPRISTONA ….. • Bloquea el efecto de la progesterona natural sobre el endometrio y la decidua. • Aumenta la separación de las fibras en el miometrio. • Actúa sobre el aparato reproductivo al antagonizar directamente la producción de gonadotrofina a nivel de la hipófisis.

CONTRACEPCION POST-COITAL Se ha reportado, incluso, mejores resultados usando Mifepristona, que con los contraceptivos orales combinados o con el Danazol. “Webb, A.M.C. et al Comparison of yuzpe regimen, Danazol, and Mifepristone (RU486) in oral post-coital contraception.   13. ABORTO TEMPRANO Esta indicación ofreció la primera oportunidad de probar la acción del RU486 en humanos. En 1991 realizamos un estudio en colaboración con el Population Council, en aborto temprano, utilizando la Mifepristona + Prostaglandina. “Winikoff B. et al.   14. ABORTO EN EL SEGUNDO TRIMESTRE El uso de Mefepristona disminuye el nivel de prostaglandina necesaria para completar el aborto y acorta el tiempo entre la administración del medicamento y la producción del mismo.

  15. MADURACION DEL CUELLO UTERINO • La acción de la Mefepristona sobre el cuello uterino es útil para manejar situaciones tales como: • Preparación para aborto del 2do. Trimestre. • Preparación para la inducción del parto.

  16. INDUCCION DEL PARTO Varios estudios demuestran los efectos favorables del uso de la Mifepristona para la inducción del parto. Incluso acortando en 24 horas este inicio cuando se compara con placebo. “Frydman R. et al.   17. OTROS USOS TERAPEÚTICOS • En la endometriosis. • Leiomioma uterino • Cáncer de mama • Hasta el momento no existen evidencias sólidas sobre ventajas del uso de la Mefepristona, frente a otros tratamientos utilizados para estas entidades.

  18. Primeras publicaciones “Píldora abortiva”. • Reacción en contra de los opositores al aborto. • Retrasaron el desarrollo de investigaciones no sólo en el campo de la reproducción humana sino, incluso, en otros campos de la • Medicina.
