  7. Accelerated Bridge Program, Phase 1B General Scope of Work • 48 Bridges Total • Primary Work: Bridge Deck Replacement including: • Approach Slabs • Sidewalks • Curbs • Bridge rails • Joints • Asbestos Removal • Maintenance of Traffic and Work Zone Protection

Accelerated Bridge Program, Phase 1B General Scope of Work (cont.) • Other Work includes: • Bearing Restoration and/or Replacement • Approach Guiderail Replacement • Pedestal Repair • Miscellaneous Steel Repair • Backwall Repair and Modifications - to eliminate abutment joints (make approach slabs continuous with deck) • Utility Coordination • Other Miscellaneous Work Load rating after rehab shall be HS-20 (36 Tons)(may require steel modification)

Accelerated Bridge Program, Phase 1B Quality Control (QC) • QC is Design-Builder’s Responsibility • Design-Builder shall appoint a QC Manager responsible for QC Plan for both Design and Construction • Design QC shall be by the Designer in accordance with the QC Plan • Construction Inspection shall be by an Independent Engineering Firm (Hired by the Design-Builder) • Materials Testing shall be by an Independent Testing Firm or Laboratory (Hired by the Design-Builder) • The Independent Testing Firm shall report to the Construction Inspection Firm who shall report directly to the Department • Offsite Plant Quality Control will be performed by the Department

Accelerated Bridge Program, Phase 1B Quality Assurance (QA) • QA is Department’s Responsibility • The Department may perform QA on any of the Design-Builder’s activities • NYSDOT Construction Quality Assurance Engineer on each Contract • The Department will utilize an independent Consultant to perform QA of the Design-Builder’s Designs • “Over the Shoulder” Design Reviews

Accelerated Bridge Program, Phase 1B Project Schedule • Project Schedule • Total Duration 18 Months +/- • Percentage of Bridges to be completed by November 2012 • All Construction Work to be completed by November 2013 • Liquidated Damages may be imposed for missed completion dates for each bridge. • Additional Liquidated Damages may be imposed for missed completion dates for all bridges scheduled in a season.
