PDO Incident First Alert - PDO / GEO CHEM What happened While boarding a vessel during moderate sea condition, as the pilot boat approached the pilot ladder when the sea dipped causing, the inspector lost his balance and fell into the water. He was immediately rescued and evacuated to PDO MAF clinic. After preliminary examination and treatment he was referred to KIMS where he was diagnosed with fractured right shoulder. PDO Incident First Alert - Oman Shapoorji Company What happened While working on rebar bending activity at the steel workshop, the steel fixer bruised the tip of his left hand ring finger and received first aid onsite. He later felt dizzy then fainted and fell to the ground striking his head causing a fracture to the skull. PDO Incident First Alert - PDO What happened While moving a 13m pipe through a workshop gate, one end of the pipe hit an existing stand moving the load on the sling causing the pipe to drop on to the left foot of the technician assisting the movement. PDO Incident First Alert - HAL Service Halliburton What happened • The driver observed that the truck was not performing properly, he stopped the vehicle to check the condition of truck and trailer. Whilst doing a 360-degree check around the vehicle, the tire caught fire and burst ejecting debris causing injury to his eyes. Mr. Musleh asks the questions of can it happen to you? Do you ensure Heavy Vehicle brake air test is conducted prior to start a journey? Do you report any issues with brakes immediately to maintenance? He was treated immediately by a first aider on scene and transferred Khoula hospital.   18. PDO Incident First Alert - Medco LLC What happened While a trailer was passing over a pipeline, the crane operator placed his foot on top of the pipe in an attempt to steady it. As the trailer moved the pipe struck his leg resulting in a fracture to his right leg.
