  3. Aan-/afmelding Aanmelden nieuwe patiënt / afmelden patiënt • Bij voorkeur d.m.v. faxformulier naar SHO • M.b.v. mutatie op maandelijkse ‘controlelijst’ van SHO ‘Nieuwe patiënt’ = • nieuw gediagnosticeerd (< 3mnd foto) • overgekomen van andere praktijk

  4. Controle • SHO stuurt praktijk maandelijks controlelijst • Praktijk controleert lijst: • patiënten verhuisd of overleden? • om andere reden geen fundusfoto nodig? • evt. nieuwe patiënten hierop aangeven • Praktijk faxt lijst met of zonder aanpassingen retour SHO • SHO stuurt overzicht met ‘non-responders’

  5. Uitvoering (1) • Jaarlijks fundusfoto • SHO stuurt oproepbrief + informatiefolder aan patiënt (oproep niet gekoppeld aan geboortedatum) • SHO voert fundusfoto uit op locatie • Patiënt kan keuze maken voor andere datum en locatie

  6. Uitvoering (2) • < 3 maand screening zodra melding bij SHO is ontvangen • Spoed screening < 1 week mogelijk (locatie?) • aanvragen m.b.v. faxformulier • Extra screening mogelijk, bv bij instellen insulinetherapie • aanvragen m.b.v. faxformulier

  7. Motivatie! Belangrijk dat huisarts, POH en assistente patiënten informeren en motiveren! NB: no-show ca. 15% !

  8. Beoordeling en rapportage • Beoordeling foto door oogartsen van Alysis • SHO rapporteert uitslag: • in HIS en Portavita • en/of op papier • ook de uitslag ‘niet geweest’

  9. Vervolg N.a.v. de uitslag: • patiënt wordt jaarlijks opgeroepen of • patiënt wordt uitgeschreven wegens verwijzing naar oogarts SHO archiveert de uitslagen Desgewenst kan oogarts CD-rom met foto’s ontvangen

  10. Hoe gaat het in zijn werk? • donkere kamer • non-mydriatic camera • ca. 15% mydriasis • 2 foto’s van beide ogen • duur onderzoek ca. 10 min.
