Healthy Heart ProgramStep One Your weight, blood pressure, cholesterol and glucose numbers provide a snapshot of your overall health and can help identify the warning signs of many serious illnesses. When you take the health questionnaire (Step 2), you'll be asked to provide personal numbers, like weight and blood pressure. You don't need these numbers to take the health questionnaire or qualify for rewards, but you'll receive a more accurate result if you do. 4. Healthy Heart ProgramStep Two To begin participating, you must complete the health questionnaire. The online tool will lead you through a series of questions, and will identify possible risk factors for developing cardiovascular disease. You’ll receive a personal report highlighting your risk factors, goals and an action plan to help you make positive lifestyle changes. Your coach can support you in making healthier lifestyle changes – lose weight, improve diet, exercise more, reduce stress, reduce blood pressure, reduce cholesterol, quit tobacco, and more. Talk to a Coach.
