Rationale: Lord Dazi Report - ‘High Quality Care For All’ The NHS should ensure that health care is personalised and fair, there is access to the most effective treatments, that systems are safe and that patients are helped to stay healthy. Quality is at the heart of the NHS and front line staff should be enabled to initiate and lead change that improves quality of care for patients.

Improved hospital mortality rates • Based on CHKS data the Trust has an HSMR of 103 (2007/8), compared to the average of 100 • Goal: to achieve a HSMR of 73 by 2011/12
Conformance to best practice We want to ensure that our patients receive healthcare proven to improve their outcomes Goal: to implement Advancing Quality measures and maintain our performance in relation to 100,000 lives and Saving Lives campaigns by 2008/9
Reducing avoidable harms Research shows 10% of patients will be subject to an avoidable harm. Goal: to reduce an estimated 8,500 adverse events a year by 50% by 2011/12
Improving the patient experience We achieve only average ratings from patients, in the annual Patient Satisfaction Survey for our services Goal: improve satisfaction rating for the quality of patient services by 5 points per year, with the aim of achieving top 10% performance for all indicators by 2011/12
How we will deliver our goals The ‘Blackpool Way’ aims to create a fully engaged workforce where individuals and teams have greater influence and autonomy in driving the Trust towards best in class performance. This will be key to us achieving our goals as we aim to use a continuous improvement approach to deliver change in each of the areas identified.
