Common errors • Legal names vs. “Nicknames” • Incorrect Social Security Numbers • Incorrect date of birth • Forms missing signatures and dates • Information on forms don’t match what is entered into SCORE • Missing Prior Service Reference • Missing documents • Documents uploaded under wrong tabs • CPS history not thoroughly assessed • Incomplete sections in Home Study • Missing staff and director/designee signature • Missing attestation statement • Missing required references • CPR/First Aid and IMPACT dates not entered • Lacking a thorough assessment of the family • Missing Confirmation Statements • Approving homes prior to the return of CPS, Safety Checks, and CPR/First Aid
Common errors cont. • Missing Bright From the Start Compliance Report for homes with licensed In-home Daycares • Form 44 not updated • Missing assessment of CPS History to include screen outs • Re-evaluation mirrors previous year (i.e. no changes) • Failure to document and verify sleeping arrangements • Failure to Interview all household members to include the foster children currently placed. • Continued Parent Development hours not listed in Narrative or SCORE • Continues Parent Development hours not relevant to the placements • Indicating whether or not Carbon Monoxide Detectors are on sleeping levels • Description of pets and vaccination dates
