Approximately 20,000 Mississippi Medicare beneficiaries will be eligible to receive disease management services
Mississippi is one of nine states chosen for the three-year CMS pilot program
Must have a diagnosis of heart failure and/or diabetes
Primary objectives:
Improve clinical quality
Improve beneficiary and provider satisfaction
Achieve overall net cost reduction of at least 5%
Develop new strategies for chronic care management

4. Evolving population care management model
5. Chronic Care Improvement Program Physician-directed delivery model to promote and sustain the beneficiary-physician relationship
Quality improvement/pay for performance incentives
Nurse support available to physicians and beneficiaries to reinforce the treatment plan
Community-based nurses; 24/7 nurse call center.
Remote health monitoring/home monitoring devices

Review reporting tools and quality improvement efforts
Recruit local physician champions
Providing assistance in educational materials; Training staff in QIO performance measures
Participation in technical consulting group
Support use of COMMAND software
