This program aims to increase youth's knowledge of Katishtya culture and traditions while building resilience through bonding with elders and promoting social responsibility. The program also adds value to the quality of life for elders and teaches core values that have been passed through generations.

Where do I start… Identify your resources Work with Health and Wellness Programs/Behavioral Health Programs Identify individuals interested in sharing culture and tradition ask them to part of the process Engage elders and get their input Identify funding sources that can support activities through their stated scope of work

Pueblo of San Felipe Structure The Pueblo has 13 programs that provide services to the community. Oversight provided by: • 3 Tribal Administrators • Tribal Administration • War Chiefs • Governors • Fiscales Those who serve in the capacity under the Tribal Administration become lifetime Council Members.

Pueblo of San Felipe health & wellness program Clinical Services • Health Clinic – IHS operated • Dental Clinic – Indian Self-Determination and Education Assistance Act PL 93-638 • Public Health Nurses/Community Health Representatives • Dieticians/Nutritionist • Fitness Programs • Diabetes Prevention Programs
Outcomes of effective coordination • Regular interaction and exchange with the Health Board • Regular monthly health screenings at the senior center • Information exchange with PHNs and CHR’s regarding health and well being of elder clients • Regular communication with pharmacist for medication management and safety • Access to benefit services and assistance; Medicaid/Medicare assistance • Efficient referral process which allows access to other benefits outside of clinical services which support health and wellness
Impacts of Effective Coordination • Noticeable improvements to the health status of elders within the community • Increase in elder participation in programs and services offered • Comfortability of caregivers and families to be more proactive and involved in the health care of elders • Involvement in planning of future health center for the community • Enhanced referral process which accommodates elders and families and ensures “wrap around” care • Healthy aging awareness through out the community
