Report Information • Performance Report – gives details of your agency’s achievement on indicators for the past period of July 1, 2012 through June 30, 2013.
Basic Information • www.hwmuw.org • Click “Our Work” tab, “Agency Providers”, “Resources for Agencies” and then “United Way’s Impact Center” • OR use this direct link: www.uwimpactcenter.org • Usernames and passwords are the same as last year. • Select the Performance Reports tab at the top of the screen. • Select the report to be completed by it’s due date: • Performance Reports are due July 31, 2013

Navigating within Reports • “jump to section” – allows you to save the current page and move on to the selected section. • “Save changes” – saves the report with it’s current information and you remain on the current page. • “Save and exit” – saves the report with it’s current information and takes you out of the report. • “<<“ and “>>” – saves the current information and takes you forward or backward to the next section. • The report must be saved BEFORE exiting or moving on to a different report or you will lose your data! • Be sure to SUBMIT the report when you have completed the revisions: Submit Proposal
Tips I. Parameters: • Indicators did not receive United Way funding will show a $0. • Did you complete a 2-1-1 update survey in the last 12 months? • If you are unsure if your agency has completed the update, contact John Mitchell at #752-8648 or jmitchell@hwmuw.org . • Projected vs. Actual number of individuals served.
Tips II. Achievement: • Data is pre-entered based on the Revision Document that you completed on August 15, 2011. • If Proposed data differs from Actual data in any section, use the narrative sections to explain or correct. • Please be as detailed as possible when explaining changes. If no changes were made to the program, simply state No Changes in the narrative box. • If you do not know if a funding recommendation was given, contact Emily Schichtel.
Tips IIIa. – IIIe. Demographics: • Make sure all boxes have a GREEN CHECKMARK. This means data you entered is consistent with previous parts of the document. • A RED CIRCLE means data is not consistent and you will have to find and correct your error. • In section IIIe. explain why some demographic data is not collected or reported, especially if it is a significant percentage of the population served.
Tips • Use the pull-down menu to select “Revision Review and Submission.” This view will allow you to view the entire report, print a hard copy and track the number of the report (document identifier). • Using this screen, you can go back to any section by clicking on [go back to this step]. • Be sure to SUBMIT the report when you have completed the revisions: • System will lock down after 11:59pm July 31, 2013.
