  3. Step 1: Review All PSPC Informational Webinars • Webinars are located at www.healthcarecommunities.org • Each webinar focuses on a different topic. Topics include: • What is PSPC?: An Overview • How to Form a PSPC Team • A PSPC team success story • How to Enroll in PSPC 6.0 • Collaborative Expectations How to enroll

  4. If you are a team from PSPC 5.0… • There is no need to re-enroll in PSPC 6.0 • Teams participating in PSPC 5.0 will automatically be enrolled in PSPC 6.0 How to enroll

  5. Step 2: Complete the On-Line PSPC Participation Package (new teams only) • Visit www.healthcarecommunities.org • Click on “Join Today” How to enroll

  6. Step 2: Complete the on-Line PSPC Participation Package • Complete the information on the Participation Package Link • Share contact information for all teams and partners How to enroll

  7. Step 3: Welcome E-Mail • PSPC will review submitted information and send a “welcome” email to the team lead. A copy of the welcome email will also be sent to the QIO that recruited the team (if applicable). • Team Leads should share information with all team members, partners, and senior leaders • Your welcome email will provide your team with “next steps!” How to enroll

  8. Open Enrollment Process • PSPC has an Open Enrollment Process • Teams can enroll throughout the collaborative year • Teams are encouraged to enroll in November and December or as early as possible to take advantage of all offerings during the collaborative year. How to enroll
