Aim of the project To help London Boroughs improve the transition pathway by: • Accurate demand forecasting through data collection • Identification of local solutions for people with complex needs and high cost placements • Advice on expectation management • Network support for learning disability and personalisation needs • Practical steps for individual/family/professional partnership to manage change to personalised provision • Tool to shape and manage the market
Data collection and analysis • Questionnaire to all 32 boroughs • Learning disability commissioner or lead • Emerson data on prevalence for each borough • Asked to compare to own data • Asked to describe how they collect and use data
Responses • 19 supplied • 11 promised • 2 ‘no shows’
What the responses so far show from the adult side • Prevalence on severe and profound learning disability broadly understood • Moderate learning disability and autistic spectrum disorder present difficulties • Some boroughs very much on top of this and have simple joined up systems • Some are struggling to get data • Some are struggling with data • Some send it off to children’s services
