Outcomes • Be familiar with the types of postural pain syndromes. • Be familiar with the main pattern of muscle imbalance in the cervico-thoracic area. • Be familiar with the most widely used physiotherapy treatment protocol for a patient with a typical postural pain syndrome.
Postural Pain syndrome • Prof M Roccabado: thumb-sucking during childhood and the biting of pens in later years were reasons for cervico-thoracic postural pain syndromes • Encourages rounded shoulders and ‘poking-chin” posture
Syndromes • Postural overload • Postural overexertion • Vibration syndrome • Postural pain
Muscle imblance (Janda) • Shortened m. levator scapulae, upper fibres of m. trapezius and m. pectoralis • Weak deep neck flexors (suprahyoid and mylohoid) • Weak scapulae stabilisers (m. serratus anterior, m. rhomboids, mid and lower m. trapezius) • m. sternocleidomastoid is also shortened
Treatment • Mobilise stiff joints where the capsule and ligaments are shortened • End of range Grade III and IV as well as sustained stretch • Stretching of shortened muscles • Strengthening of weakened muscles • Activating of stabilisers • Re-education of posture • Home advice
