WHATIS A CBO • A “COMMUNITY-BASED ORGANIZATION“ MEANS A PUBLIC OR PRIVATE NONPROFIT ORGANIZATGION OF DEMONSTRATED EFFECTIVENESS: • IS REPRESENTATIVE OF THE COMMUNITY OR SIGNIFICANT SEGMEMTS OF A COMMUNITY • PROVIDES EDUCATIONAL, SOCIAL SERVICES OR OTHER RELATED SERVICES.
GOALS OF A CBO • Assess and provide the needs of the community • Organizing events and allowing participation from residents of the community • Making a positive difference in the community • Establishing relationships with other providers in the community that align with their mission. • Provide affordable housing • Provide sustainability in the community (i.e. food pantry, clothing etc.)
WHY COLLABORATE? • Provide a more holistic approach to care and enhances the quality of care. • The movement from fee for volume to fee for value is at the foundation of the evolution in health care today. CBOs, payers and health care providers have to understand that in order to flourish in this new world will require partnership built on new business models, shared goals and transparent collaboration. • Cut cost, easy access to care, improves coordination.

The AIDS Hospice Foundation was developed from the Catalyst Organization, the Los Angeles AIDS Hospice Committee co-founded by the earliest champions of the AIDS Hospice Movement, Chris Brownlie, Michael Weinstein, Sharon Raphael, PhD, Mina Meyer, MA, and other advocates. 25-Bed Hospice Opens Barlow Respiratory Hospital converted into Chris Brownlie Hospice, providing 24-Hour Medical and Palliative Care to people living through the Final Stages of AIDS. Case Management Definitions: Case management is a multi-step process to ensure timely access to and coordination of medical and psychosocial services for a person living with HIV/AIDS and, in some models, his or her family/close support system. Case management includes the following processes: intake, assessment of needs, service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment, case conferencing, crisis intervention, and case closure. Case management activities are diverse. In addition to assisting clients to access and maintain specific services, case management activities may include negotiation and advocacy for services, consultation with providers, navigation through the service system, psycho-social support, supportive counseling, and general client education. The goal of case management is to promote and support independence and self-sufficiency.
The processes of case management described here are: I. Brief intake/assessment process II. Selection of case management model and placement III. Brief Service plan IV. Initial comprehensive assessment V. Initial comprehensive service plan development VI. Service plan implementation, including client contact, monitoring and follow-up VII. Reassessment VIII. WHAT CAN A CASE MANAGER DO FOR THE PHYSICIAN • Helping clients retain or stay in care and provide appointment reminders • Connecting clients to care and referrals needed • Helping stabilize the client after a crisis or hospitalization • Addressing social determinants of health that affect their ability to participate in care. • Share a patient centered plan with you that includes client’s goals, and interventions and tasks the clients have adhered to. • Share information collected in a comprehensive assessment that will help you provide a better care for your clients. • Case Managers can provide ongoing reinforcement regarding medication regimens, attendance to specialty care. • Arrange transportation to and from the hospital
