SPNS Program HRSA’s Ryan White HIV/AIDS Program (RWHAP) Part F Provides opportunities for the development, implementation, and assessment of system, community, and individual-level innovations designed to: • Meet the National Goals to End the HIV Epidemic • Address the demands of changing health care delivery systems  • Quickly respond to emerging needs of clients served by the RWHAP 69 current SPNS grant recipients Provides clinical and supportive services to over 9,000 individuals living with HIV Promotes dissemination and replication of successful SPNS models

Featured SPNS Initiatives Enhancing Engagement and Retention in Quality HIV Care for Transgender Women of Color (2012-2017) • 9 demonstration sites funded to design, implement, and evaluate innovative interventions to successfully engage and retain in care transgender women of color at high-risk of or infected with HIV Building a Medical Home for Multiply Diagnosed HIV-Positive Homeless Populations (2012-2017) • 9 demonstration sites funded to design, implement and evaluate innovative interventions focused on the development of sustainable linkages to care for homeless or unstably housed PLWH with co-occurring mental illness and/or substance use disorders Innovative Methods for Integrating Buprenorphine Opioid Abuse Treatment in Primary Care (2004-2009) • 10 demonstration sites funded to integrate buprenorphine opioid abuse treatment into HIV primary care settings to improve health outcomes for PLWH with opioid dependency
