50 Hoops Corporate • 50 Hoops is a 501 ( c ) (3) organization with (2) components: • The National 50 Hoops Basketball Tournament and Health Fair • The National Physician and Family Referral (NPFR) Project • The 50 Hoops Health Fairs also include NPFR recruitment. • NPFR is a national physician, community and media outreach for African American prostate cancer patient & family recruitment. • 50 Hoops' function includes administration and recruitment for NPFR. 50 Hoops Tournaments & Health Fairs The 50 Hoops Tournament & Health Fair includes the following components: • Prostate Awareness Basketball Tournaments for men 50 and over • Prostate Education (and Quality of Life) Health Fairs • On-Site Free Prostate Cancer Screenings with local collaborative follow-up • Prostate & Breast Cancer Awareness conferences and workshops for lay and professionals • Coordination of on-site support and recruitment for Prostate Cancer research and human genome studies • Slam Dunk Prize Box: The KEY to winning prizes is to attend and participate
50 Hoops Events Overview 50 Hoops is reaching a greater percentage of the underserved men than any other prostate cancer education program through our National Tournament and Health Fair. Education + Participation = Saved Lives Our purpose is to raise awareness about prostate cancer. Research, health education and early diagnosis is our focus. We serve as a national educational (fun) source for prostate cancer and health awareness. 50 Hoops is also a consumer information vehicle for participating pharmaceuticals, product sponsors, health and fitness professionals. Our goal is to educate, and recruit African American Families and to screen men 40 and over who may not otherwise have resources to get screenings and follow-up services.
National Physician & Family Referral (NPFR) Project FIND THE CAUSE of Prostate Cancer through Genetic Research The NPFR Project is a new 50 Hoops acquisition Project Mission: Instill public awareness of prostate cancer research and recruitment of families who meet the criteria for national and international human genome research studies. Serve as community and media advocates and liaison for a national and international physician, community and media recruitment of African American families (and those of West African descent) who meet the criteria for human genome research studies. Project Goal: To identify prostate cancer survivors and family members that will act as "contacts" for the families in this human genome recruitment effort. To identify families who have family members (father, brother, uncle, cousin grandfather) who may have been diagnosed with prostate cancer and who meet the criteria for national human genome studies focusing on African Americans.
Pilot Project MissionBegan 2001 Initially, to identify (up to 50) African American families (beginning in Northern California; expanding to Southern California and West Coast) who meet research criteria for African American Hereditary Prostate Cancer (AAHPC) Study of 4+ affected by the disease. To identify networks within the medical community for locating African American man/families with history of prostate cancer within the Western Region. This project supports, expands and enhances the AAHPC Study objectives to find 100 families who meet the criteria.
