Eligibility Criteria Aged 65 and over Must live in the City of Sunderland – All tenure eligible Must have a support need to: Develop a stable lifestyle Access other appropriate services Access health services Live safely & securely Become socially included with friends/family and/or their local community Be willing to work with staff on an outcome based plan that defines future aspirations

Gentoo Customer Findings Since 1st Oct 2012 We have: Actively identified 3875 potential customers Contacted 2662 of those potential customers Recorded a 28% acceptance rate of a wellbeing service Examples Of Outcome Feedback From Customers Who Received The Service: Felt safer – 23 Improved ability to manage unassisted – 22 Improved independence - 8 Prevented future falls – 6 Better off financially – 5 Increased social contact - 2
