  3. Only need when program receiving CME credit through the MTN Left side of Instructor folder File after CV Form (3/4)

  4. Copies of Additional CertificationSubject Matter of Expert • Examples: • NREMT-B • RN • MD Left side of Instructor folder. File after Financial Disclosure Form(4/4)

  5. Right Side of Instructor Folder All info must fill-in. This only applies for PD of the Training Site All info must fill-in. This only applies for PD & TSF of the Training Site Right side of Instructor folder On top(1/6).

  6. All info must fill-in & Signed By the Training Site’s PD Right side of Instructor folder. File after TSF or PD’s Form (2/6) NOTE: First time instructor will not having this form.

  7. NOTE: All Instructors must be monitored before issuance or re-issuance of Instructor card. All instructors must monitor at least once every 2 years or upon assignment to a new TS. Right side of Instructor folder. File after Instructor Renewal Checklist(3/6).

  8. This form must completed & Signed by candidate & TS’ PD Right side of Instructor folder. File after Instructor Monitoring Form(s)(4/6).

  9. Right side of Instructor folder File after Instructor Candidate Form(5/6). Copy of signed wallet card(front & back)

  10. Certificates or memorandums of training for in-services and any additional training directly related to MTN Resuscitative Medicine Training. Examples: Core Instructor Course Guidelines update Memorandum of Equipments/Disinfection training. Right side of Instructor folder On bottom(6/6).
