  4. What is Health and Wellness? Illness, injury and preventive health care

  5. Services provided through the Health Fee www.snc.edu/health/clinicalservices.html RN assessments and tests for strep, mono, influenza, iron, glucose, etc. General health promotion practices and strategies Counseling services Services with a charge are billed to insurance Medical services: Physician Assistant or Nurse practitioner on campus daily Immunizations, prescriptions, procedures, durable medical goods Transportation to health appointments Metro Bus (free) Cab Vouchers (cost billed to student acct.) Clinical Health Services

  6. SNC Health Requirements • 1. Health History Form, 2. Immunizations 3.Health Insurance https://stnorbert.medicatconnect.com/ • Students login with their SNC network username (last 4 letters of last name, first and middle initials) and the password they created for their email account (e.g. Mary Ann Jones network login is jonema) • Student’s Health History Information regarding past health/mental health conditions, allergies, medications and any ongoing treatment (provide plan of care) http://www.snc.edu/health/docs/PlanofCareForm.pdf

Required immunizations • Measles, Mumps and Rubella, MMR (2) • Tetanus/diphtheria,/pertussis, Tdap (1) • Additional Immunizations • Hepatitis B (3) • Meningitis (Menactra, Menveo); A booster dose is recommended if initial vaccination was given before age 16 • Include all other immunizations received, especially any related to travel outside of US
