Example: Early Identification Clinic • What • offers families with children aged 0-6 w/ one day and location for full-circle check-in of children’s health. • Why • 70 % of brain development occurs before the age of 5 • Lack of access to/ service gaps in early child check-up services by vulnerable families. • Outcomes • Screening: Developmental or other health issues identified in approx. 3 out of 20 cases.
